The Royal Voluntary Service is a national volunteer organisation that enriches the lives of older people and their families across Britain. The overriding aim is to provide practical support that enables older people to lead independent and fulfilling lives. Through the power of volunteers, the ambition is to reach two million older people by 2023.

Home from Hospital

The project provides low-level practical support to older people returning home from a stay in hospital. Offering a free six-week service tailored to the needs of the individual, our emphasis is on helping people to regain independence, reduce isolation and increase involvement in the community. Designed to be short-term and practical, the service offers transport to appointments, lunch clubs and social calls, and volunteer befriending to isolated older people, as well as signposting to a range of local services.

The service is delivered through volunteers and staff who work closely with a range of partners across the city. Referrals to Home from Hospital are generally received from health and social care professionals based in hospitals, with smaller numbers being self-referred, the local authority or through partner organisations.

This project is being delivered Citywide.
